Grants Management provides a range of materials for organizations interested in familiarizing themselves with the Statewide Financial System. These include topical documents related to Registration and Prequalification.

Registration Documents

The following documents are available to assist with the Registration process.


    Substitute W9 Form

    If your organization does not already have a New York State SFS Vendor ID, download, complete, and attach the Substitute W-9 Form to the completed Registration Form for Administrators and the Grants Management team will obtain one on your behalf.



Prequalification Documents

The following documents are available to assist nonprofits with the Prequalification process.


    MWBE Board Resolution Template

    When a nonprofit is completing their prequalification application, one requirement is to have policies/resolutions in place, including one for MWBE. This template can be used if a nonprofit organization does not have a MWBE policy.
